This form is used in El Dorado Superior Court to tell the judge whether both sides in a case have agreed to try mediation or another form of dispute resolution, or whether they disagree about using it. Either party in a civil case—such as someone involved in a lawsuit over money, property, or contracts—would need to fill out this form to state their position on alternative dispute resolution.
