The PR-20 form is used to officially ask the court to appoint someone as a legal guardian for a child or incapacitated adult who cannot care for themselves. A person would need this form if they want to become the legal guardian of a minor child whose parents cannot care for them, or of an adult who is unable to manage their own affairs due to age, illness, or disability.
