The DV-150 form is used to set up court-ordered rules for when a parent or guardian can visit with a child, requiring a neutral third party to supervise the visits and exchanges to keep everyone safe. This form is needed by people involved in domestic violence cases where the court believes direct contact between the visiting parent and the other parent (or child's custodian) poses a safety risk.
