Declaration Regarding Notice and Service of Request for Temporary Emergency (Ex Parte) Orders

This form allows someone to declare to the court that they properly notified the other party (or explain why they couldn't) about a request for emergency temporary orders, or to ask the court for permission to skip this notice requirement. Someone filing for an emergency restraining order, custody order, or other urgent court protection uses this form to show the judge that proper notice was given or to request emergency relief without waiting to notify the other side first.
