This form is used to tell the court that you have notified the other person involved in your case (or explain why you couldn't notify them) before making an urgent request to the judge without them being present. Someone would need this form when they want to ask a judge for an immediate decision on something important in their case and need to document that they gave the other side a fair warning or have a good reason for not doing so.
