This form is used in child abuse or neglect court cases to ask the judge to assign a Court Appointed Special Advocate (CASA) — a trained volunteer who investigates the child's situation and gives recommendations to the court about what's best for the child. A parent, social worker, attorney, or other person involved in a child welfare case would file this form when they believe an independent adult advocate is needed to look out for the child's interests.
