# FOAH-06 Purpose This form is used to provide detailed information about a child's living situation, parent schedules, and what arrangement would be best for the child when parents are asking the court to decide custody and visitation matters. A parent or guardian who is filing a case in Kern County Superior Court to establish or change who has custody of a child or how much time each parent spends with the child would need to complete this form.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
