The Juror Questionnaire is used to collect personal information from potential jurors so the court can determine if they are qualified and unbiased enough to serve on a jury for a case. Anyone who receives a jury summons from Madera Superior Court would need to fill out this form to help the judge and lawyers decide if they should participate in a trial.
