This form is used to show a court that you have officially notified all required people (like family members or interested parties) about guardianship proceedings by delivering them copies of court documents. Someone going through the guardianship process—such as a person petitioning to become a guardian of a child or an adult—would need to complete this form to prove they followed the law's notification requirements.
