This form allows a veteran in the San Mateo County court system to give permission for their medical, mental health, and other personal records to be shared between the court, treatment providers, and other agencies involved in their case. A veteran who is participating in the Veterans Treatment Court program would need to sign this form so that the judge and treatment team can access the information necessary to help them address underlying issues like PTSD, substance abuse, or trauma instead of just facing criminal punishment.
