The JV-665 form is used by a judge to document the final decision and sentence in a juvenile delinquency case after a youth has been found guilty or has pleaded guilty to a crime. A juvenile (a young person under 18) who has been charged with a crime in Imperial County Superior Court would need this form filed in their case.
