The NC-100 form is used to ask a California court to officially change your legal name to a different name. You would need this form if you want to legally change your name for any reason, such as after marriage, divorce, personal preference, or to correct a mistake on your birth certificate.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
