The Grand Jury Questionnaire is used to collect background information from people who are being considered for selection to serve on a grand jury, which is a group that reviews evidence to decide whether criminal charges should be filed. Anyone summoned by the court who is a registered voter, U.S. citizen, and meets other basic eligibility requirements would need to complete this form.
