# Order for Modification of Probation This form is used to ask a judge to change the terms or conditions of someone's probation—such as adjusting the length of probation, changing reporting requirements, or altering other rules they must follow. A person on probation (someone serving a sentence in the community instead of in jail) or their attorney would use this form when circumstances have changed and they need different probation rules.
