Stipulation for Appointment or Removal of Mediator

The FL-18 form is used by both parties in a family law case to agree together on hiring a mediator to help resolve their dispute, or to remove a mediator they've already hired if they're no longer satisfied with them. People going through a divorce, custody battle, or other family court case would use this form when they and their spouse or co-parent mutually agree on who should mediate their disagreement.
