Form L-1061 is used by a creditor or debt collector to notify the court that a defendant has failed to make required payments on a judgment or that the defendant has partially paid off the judgment amount. A creditor who has won a lawsuit and obtained a money judgment would use this form to formally document payment problems or incomplete satisfaction of the debt.
