Findings and Orders Regarding Prima Facie Showing on Nonminor's Request to Reenter Foster Care

The JV-470 form is used to document a judge's decision on whether a young adult (ages 18-21) has proven they meet the requirements to re-enter the foster care system after having left it. A young adult who has aged out of foster care or left the system would need this form to formally request to come back under court supervision and receive support services.
