Supplemental Petition for More Restrictive Placement (Attachment) (Welf. & Inst. Code, § 387)

This form allows a social worker or attorney to ask the court to move a child to a more restrictive living situation, such as from foster care to group home or institutional care, when the child's current placement is no longer safe or appropriate. Child protective services agencies, dependency counsel, and parents' attorneys use this form during ongoing dependency cases when they believe a child needs a higher level of supervision or care for placement change.
