DV-109 is used to request a protective order (restraining order) that prevents someone from harassing, threatening, or abusing you, and it's filed in court as part of a domestic violence case. You would need this form if you're experiencing abuse from a spouse, ex-partner, family member, or someone you live with and want the court to legally order them to stay away from you.
