San Mateo Superior Court · Adoption
Verified current as of July 19, 2026
This form is used to confirm that an interpreter was present in court and swore to accurately translate everything said during a legal proceeding. Someone involved in a court case who doesn't speak English fluently would need this form to have an official record that their communications were properly interpreted.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
