# CR-004 Explanation Form CR-004 is used by people who have been convicted and sentenced to ask the court to take back or change their sentence because of new laws or changed circumstances. Someone would need this form if they believe their punishment is no longer appropriate under current law or if there are new reasons the judge should reconsider their sentence.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
