The MFD-100 is a cover sheet that goes on top of a stack of court documents to organize and identify all the papers being filed together as one package. Someone filing papers with the court themselves — without a lawyer — would use this form to make sure the court clerk knows what documents are included and can process them correctly.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
