The FL-311 form is used to provide detailed information about a child's living situation, parents' work schedules, and the proposed custody and visitation arrangement when someone is filing for custody or visitation rights in California court. A parent, grandparent, or other relative who wants to establish or change who has custody of a child or when the child spends time with each parent would need to complete this form.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
