This form is used by one parent to ask the court to give them complete legal authority to make all major decisions for their child, such as choices about education, healthcare, and religion, while the other parent has no say in these decisions. A parent would use this form when they want sole custody and are filing in Monterey Superior Court, or when the court has already decided to award them sole custody and they need the official court document.
