This form is used when a parent or guardian needs another adult to supervise visits between a child and the other parent, typically because a court has ordered supervised visitation due to safety concerns or other family issues. A parent, guardian, or someone else with custody would use this form to set up and document an agreement about who will supervise the visits, where they'll happen, and what the rules will be.
