The Declaration of Disclosure is used in family law cases (like divorce or custody disputes) to list all assets, debts, income, and expenses so both parties know what property and money is involved. Either spouse or parent going through a family court case in Monterey County would need to fill this out and give it to the other party.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
