This form is used to ask a court to temporarily remove someone's guns or prevent them from buying guns if that person is believed to be an immediate danger to themselves or others due to recent behavior, threats, or statements. A family member, law enforcement officer, healthcare provider, or school official would use this form when they're worried that someone might hurt or kill themselves or someone else.
