This form is used to make changes to a drug offender's sentence after they complete a treatment program, allowing them to potentially reduce their prison time or have their conviction reduced. Someone convicted of a non-violent drug offense who has successfully completed drug treatment would use this form to ask the court to modify their original sentence.
