The MFW-100 is a cover page that organizes and identifies all the documents someone is submitting together to Mendocino Superior Court, listing what forms and papers are included in their court filing. Someone filing their own court case without a lawyer—such as a person starting a family law matter, small claims case, or other civil proceeding—would use this form to make sure the court clerk knows exactly what documents they're submitting.
