# Proof of Personal Service (SB-37) This form is used to show that court papers were officially delivered to another person involved in a court case, rather than mailed or served another way. A process server, sheriff's deputy, or other authorized person who physically handed the documents to the recipient would complete and file this form to prove the delivery happened.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
