The MFL-100 is a cover sheet that goes on top of a packet of court documents to organize and identify all the papers being filed with the court. Someone filing their own case without a lawyer—such as a person going through a divorce, small claims dispute, or eviction—would use this form to make sure the court clerk knows exactly what documents are included in their submission.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
