Supervised Visitation Report
Merced Superior Court · Additional Resources
Verified current as of July 25, 2026
The Supervised Visitation Report is used to document what happens during visits between a parent and child when a court has ordered that a third person must be present to supervise for the child's safety or wellbeing. A parent who has been ordered by the court to have only supervised contact with their child would need to file this form to show the court a record of those visits.
