Proof of Service of Summons (Family Law)

This form is used to show the court that a lawsuit document (called a summons) has been officially delivered to the other person involved in a family law case in Mendocino County. A process server, sheriff, or other authorized person who physically delivered the summons to the other party would complete and file this form to prove they did their job.
