This form is used to request that the court unseal and release confidential adoption records, including information about the adoptee's biological parents and adoption history. A person who was adopted, an adult adoptee, or sometimes birth parents would use this form to try to access records that are normally kept private.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
