The JV-205 form is used to establish and document a court-approved schedule for when a parent or relative can visit a child who is under the supervision of the court in a juvenile dependency case (typically because the child was removed from their home due to abuse or neglect). A parent, relative, social worker, or attorney involved in a child welfare case would use this form to get a judge's written order that clearly spells out the days, times, and conditions for visits between the child and their family members.
