Consent to Termination of Guardianship and Waiver of Service of Petition and Notice of Hearing

This form allows a guardian to end their guardianship with the agreement of the person being guarded (if they're old enough to understand) or their family, without requiring a court hearing. A guardian who wants to stop being responsible for caring for and making decisions for another person would use this form when everyone involved agrees to end the guardianship.
