This form is used by someone charged with a crime in Sonoma County to ask the court to dismiss their case if they complete a mental health treatment program instead of going through the regular criminal process. A person would need this form if they have a mental illness or mental health condition that contributed to their criminal charges and they want the opportunity to get treatment rather than face criminal prosecution.
