Request for Permanent Medical Excuse from Jury Service

The JUR-100 form allows people to ask the court to permanently remove them from jury duty because of a medical condition that makes them unable to serve. Someone would use this form if they have a serious, ongoing health problem that prevents them from sitting in court for extended periods and don't want to be called for jury duty again.
