The Conservatorship Packet is used to start the legal process of having one person (called a conservator) take control of another person's money, property, and/or medical decisions because that person cannot manage these things themselves. You would need this form if you're trying to become responsible for an adult who is elderly, severely ill, or has a serious mental or physical condition that prevents them from taking care of their own affairs.
