Notice to Court of Death
San Mateo Superior Court · Adoption
Verified current as of July 19, 2026
The PR-23 form is used to tell the court that someone involved in a probate case has died, so the court can update its records and close or adjust the case as needed. An executor, administrator, attorney, or family member handling a deceased person's estate would file this form to notify the court about the death.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
