Domestic Violence Restraining Order- Change or Terminate

This form is used to ask a judge to either change the rules of an existing domestic violence restraining order (like extending the dates or modifying which locations are off-limits) or to completely cancel the order. A person who currently has a restraining order against them would use this form if they want to modify or end the restrictions, or someone with an active restraining order protecting them might use it if they want to change its terms.
