The SC-10 form is used to provide copies of evidence and documents to the other party in a court case and to formally tell the court what evidence you plan to present. A person involved in a lawsuit—either as a plaintiff, defendant, or their representative—would need to file this form to make sure the other side knows what documents or items you're bringing to support your case.

This form has fillable fields — you can complete it directly in the viewer on the right.
