This form is used to request that a court appoint someone to manage the affairs and/or personal care of an adult who cannot take care of themselves due to age, illness, or mental incapacity. A family member, friend, or social worker would use this form when an elderly person, severely ill person, or person with significant mental or physical limitations needs someone legally authorized to make decisions about their money, healthcare, or living situation.
