The LK-101 form is used by people involved in a lawsuit to prepare for and participate in a required meeting with the judge or mediator where the case will be discussed to try to reach a settlement without going to trial. Anyone who is a party to a case in Lake Superior Court (the plaintiff suing or the defendant being sued) needs to complete this form before the mandatory settlement conference.
