The SER-001 form is used to ask the sheriff's office to officially deliver court papers to a person involved in a lawsuit. Anyone filing a case or involved in court proceedings who needs proof that the other person was properly notified about the court action would use this form.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
