This form is used by someone who has already been convicted and sentenced to ask the court to change their punishment — such as reducing the prison time, adjusting fines, or modifying other conditions of their sentence. A person would need this form if they believe their original sentence was too harsh, they have completed rehabilitation programs, their circumstances have significantly changed, or they have new evidence that supports a lighter sentence.
