This form is used to finalize a divorce when the other spouse doesn't respond to the divorce papers or doesn't show up to court. Someone who filed for divorce and their spouse never answered or appeared would use this form to get the judge to approve the divorce based on their request alone.
This form must be printed and filled by hand, or opened in Adobe Acrobat.
