The Conservatorship Questionnaire is used to gather detailed information about a person who may need someone else to manage their finances and personal care decisions because they cannot do so themselves. A family member or concerned person would fill this out when they believe an elderly relative, person with a serious illness, or someone with a disability needs a conservator appointed by the court to handle their affairs.
