Consent of Proposed Guardian, Nomination of Guardian, and Consent to Appointment of Guardian and Waiver of Notice

This form allows someone to agree to become a guardian, nominate who they want as guardian, and give consent for a guardian to be appointed while waiving their right to notice of court proceedings. Parents, relatives, and other adults use this form during guardianship proceedings to either volunteer to serve as guardian or approve someone else's appointment as guardian over a minor or incapacitated person.
