This form is used to declare that someone is willing and able to become a guardian for a minor child or incapacitated adult, and it allows sensitive personal information about the proposed guardian to be kept confidential from the public court record. A person would need this form if they are stepping forward to take legal responsibility for caring for someone who cannot care for themselves, such as a relative or friend's child whose parents have died or can no longer provide care.
